Please come 30 minutes early for your new patient appointment as you will have to fill out a detailed medical history form as well as other paperwork. 

(In Progress) For a shorter time in the waiting room, please fill out the New Patient Form and email it to us before your visit.

Depending on the provider, a new patient visit usually takes 60-90 minutes including treatment. Often this includes taking full spine structural and or other x-rays as well as other imaging and diagnostic testing.

Accepted Insurance:

We are panel providers for most PPO insurances. As there are so many different types of policies, please have the staff verify your specific coverage before coming to the office for your appointment. We can also bill workers compensation, automobile medical pay, and foreign healthcare travel insurances. Additionally, our office can work with your personal injury attorney on a lein basis providing approval before your appointment.
